Our Skilled nursing care is based on dignity and compassion. We offer 24-hour nursing care with a team of dedicated professionals that provide the comfort and expertise needed for each resident’s condition. We care for:

Rehabilitation (In-Patient /Out-Patient)

Our expert rehabilitation team is here 7 days a week and committed to increase independence, ability, and functionality. We offer physical, occupational, and speech therapy care for post-acute services bridging the gap between hospital and home.

Physical Therapy

Our Physical Therapist assist with improving the mobility of our patients to be as independent as possible.

Areas of care include:

  • Ambulation training with assistive devices – canes, walkers, and crutches.

  • Wheelchair training

  • Balance training

  • Strengthening to maintain muscle function

  • Caregiver training

  • Community reintegration

  • Home assessments and modifications

Occupational Therapy

Our Occupational Therapists assist by providing care to our patients that increases their ability acclimate to the tasks of everyday life.

 

Areas of care include:

  • Functional independence - eating, bathing, toileting, homemaking

  • Maintenance of wellness - dressing, grooming

  • Prevention of further disability - home assessments and modifications, adaptive equipment– including ordering of equipment

  • Increasing safety awareness

  • Community reintegration

  • Splinting for support and muscle function

  • Adaptive equipment and training

Speech Therapy

Our Speech-language pathologists (SLPs) work to prevent, assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in adults. Our services include:

  • Dysphagia Therapy - focuses on swallowing difficulties

  • Aphasia Therapy - focuses on language disorders which occur difficulty understanding others (receptive language), or sharing thoughts, ideas, and feelings (expressive language).

  • Dysarthria Therapy - focuses on improving clarity and fluency of speech to help better communicate with others

  • Cognitive-Communication Therapy - focuses on improving skills such as organizing thoughts, paying attention, remembering, planning, and/or problem-solving, visuospatial skills, orientation, reasoning).

  • Voice Therapy - focuses on improving voice quality, loudness, resonance and tone (e.g. voice disorders, vocal fold paresis, professional voice users).

Alzheimer's & Dementia Nursing Care

Experts estimate that as many as 65% of older adults have some form of dementia, including Alzheimer's disease.

 

Caring for a person with advanced dementia and Alzheimer's disease at home is a challenging task that can become overwhelming at times. Each day brings new demands and challenges as family members adapt to changing levels of ability and behavior.  Our staff working 24/7 in our secured unit specialize in caring for all the unique challenges experienced by this special group of residents in our population.

Mission De La Casa offers a warm, caring environment in our Memory Care Unit, where we help residents’ function at their highest possible level while enhancing their quality of life. We specialize in creating individual plans of care for each resident because each resident’s needs are different.

 

Activity Department

Our activity department provides a variety of recreational and leisure experiences that are designed according to the resident or patients’ interests and life-time habits. Our Activity Director meets with each admission shortly after they arrive and assesses their preferred daily activities.  Structured activities include: exercise programs, art, music therapy, karaoke, supervised outings and one-on-one programs.

Our homelike environment includes a secure outdoor

garden-like courtyard that allows our residents and patients to freely explore the area on their own.

In addition to various activities for your loved one, we provide:
